Facilitation

Capability Resources offer a comprehensive Incident Investigation Service. Experienced personnel using the TapRooT® Root Cause Analysis, ICAM, Apollo, and various other investigation techniques can facilitate an investigation to find the root causes and recommend corrective actions for injuries, accidents, equipment failure, near misses, system failure and can facilitate improvement projects in order to stop an incident / failure before it occurs and improve productivity.

There are many instances where it is advantageous to seek external assistance from a qualified independent third party expert such as when:

  • It is likely an incident will lead to legal proceedings
  • An investigation is being conducted to resolve a customer dispute
  • An investigator is inexperienced and would gain further skills with expert assistance
  • Mediation is required for group discussions
  • An incident is controversial in nature
  • Auditing internal investigation processes to provide feedback for quality assurance
  • A business does not have the time or resources to conduct an investigation internally

Investigator Prècis

Greg Allan & Peter Berkholz are certified TapRoot Root Cause Analysis Instructors & Investigators, having conducted training and investigations across Australia, Asia and New Zealand for various industry sectors. Peter and Greg have also been trained in and used other investigation tools including Apollo, ICAM and SSAI.

TapRoot is the RCA tool of choice for many multinational companies & government departments including Rio Tinto, Exxon Mobile, Conoco-Phillips, Santos, Songa, Silcar, Woodside Petroleum, Barrick Gold, Ensign, Qantas, Australia Defense Forces and numerous others. Greg and Peter have investigated and/ or assessed numerous serious incidents including, fatalities, truck rollover, environmental incidents, collision, fire, breach of standards & legislation, near misses, engulfment, surface & underground incidents, equipment failures, electrical incidents etc.

Greg Allan

Greg Allan is the founder and Managing Director of Capability Resources (established Jan 2000) with 30 years’ experience, predominately in the mining and associated industries. Greg has worked in management roles for various companies including Rio Tinto and BHP. Since establishing Capability Resources, Greg now specialises in areas of investigation, training and safety legislative compliance having the benefit of drawing from his operational experiences over several years.

Peter Berkholz

Peter Berkholz is the Safety & Engineering Manager, Director and partner of Capability Resources (joining Capability Resources in 2004) with 27 years’ experience, predominately in the mining and associated industries. Peter has worked in management roles for various companies including Rio Tinto and BHP. Peter also worked as a Mining Mechanical Inspector for the NSW Department of Industry & Investment. Peter specialises in areas of investigation, maintenance management systems and mechanical legislative compliance having the benefit of drawing from his operational experiences over several years.
